Consensus guidelines for the identification and treatment of biofilms in chronic non-healing wounds
Abstract
Background: Despite a growing consensus that biofilms contribute to a delay in the healing of chronic wounds, conflicting evidence pertaining to their identification and management can lead to uncertainty regarding treatment. This, in part, has been driven by reliance on in vitro data or animal models, which may not directly correlate to clinical evidence on the importance of biofilms. Limited data presented in human studies have further contributed to the uncertainty. Guidelines for care of chronic wounds with a focus on biofilms are needed to help aid the identification and management of biofilms, providing a clinical focus to support clinicians in improving patient care through evidence-based medicine.
Methods: A Global Wound Biofilm Expert Panel, comprising 10 clinicians and researchers with expertise in laboratory and clinical aspects of biofilms, was identified and convened. A modified Delphi process, based on published scientific data and expert opinion, was used to develop consensus statements that could help identify and treat biofilms as part of the management of chronic non-healing wounds. Using an electronic survey, panel members rated their agreement with statements about biofilm identification and treatment, and the management of chronic non-healing wounds. Final consensus statements were agreed on in a face-to-face meeting.
Results: Participants reached consensus on 61 statements in the following topic areas: understanding biofilms and the problems they cause clinicians; current diagnostic options; clinical indicators of biofilms; future options for diagnostic tests; treatment strategies; mechanical debridement; topical antiseptics; screening anti-biofilm agents; and levels of evidence when choosing anti-biofilm treatments.
Conclusion: This consensus document attempts to clarify misunderstandings about the role of biofilms in clinical practice, and provides a basis for clinicians to recognize biofilms in chronic non-healing wounds and manage patients optimally. A new paradigm for wound care, based on a stepped-down treatment approach, was derived from the consensus statements. This article is protected by copyright. All rights reserved.
Wnt and frizzled expression during regeneration of internal organs in the holothurian Eupentacta fraudatrix
Several genes of the Wnt and Frizzled families in the holothurian Eupentacta fraudatrix are characterized, and the complete coding sequences of wntA, wnt4, wnt6, wnt16, frizzled1/2/7, frizzled4, and frizzled5/8 are obtained. The dynamics of expression of these genes during regeneration of internal organs after evisceration are studied. Evisceration and the associated damages supposedly induce the expression of wnt16 on third day after evisceration. Genes wntA, wnt4, wnt6, and frizzled1/2/7 upregulate during the period of active morphogenesis (5-7 days after evisceration) and might participate in regulation of tissue and organ formation. The signaling induced via Frizzled5/8 is could be necessary for formation of the anterior (ectodermal) part of the digestive system and development of the calcareous ring on 10th day after evisceration. Our data suggest that the Wnt signaling pathway plays a significant role in the regulation of regeneration of internal organs in holothurians. This article is protected by copyright. Giardia duodenalis et son implication dans diverses dermatoses

ObjectifAu cours de ces trente dernières années, la communauté scientifique s'est de plus en plus intéressée à la flore intestinale, microbiote ou pathogène et son impact sur les autres organes. La possibilité qu'un agent microbien intestinal soit un facteur étiologique de symptômes cutanés n'est plus à démontrer. Un parasite, en particulier, a fait l'objet de nombreuses investigations : Giardia duodenalis. L'objectif de ce travail était de dresser une revue complète des publications démontrant l'implication de G. duodenalis dans diverses dermatoses.MéthodeUne revue de la littérature a été réalisée via différentes bases de données (Medline, Pubmed et Google Scholar) pour la période de 1975 à 2015. Parmi la trentaine de cas décrits depuis 1976, nous en rapportons ici une vingtaine qui nous semblent les plus objectifs et cliniquement pertinents.Résultats et discussionBien qu'il s'agisse le plus souvent de cas isolés, cette revue démontre que la giardiose intestinale peut être associée à des dermatoses variées par l'intermédiaire de mécanismes inflammatoires, allergiques ou d'hyperperméabilité intestinale, voire en être elle-même le facteur étiologique. Les lésions cutanéomuqueuses sont variées : urticaire, angio-œdème, érythème noueux, syndrome de Wells… Le rôle et l'origine de l'infestation étant souvent inconnus, il est alors difficile de déterminer l'intervalle de temps entre l'infestation parasitaire et l'apparition des lésions cutanées. C'est pourquoi il faut discuter un examen parasitologique des selles à la recherche de ce parasite devant une urticaire ou un angio-œdème chronique ou récidivant, une dermatite atopique sévère, voire même proposer un traitement d'épreuve, tant celui-ci est aisé, et les examens de selles peu sensibles.BackgoundOver the last thirty years, the scientific community has become increasingly interested in the intestinal flora, whether commensal or pathogenic, and its impact on other organs. In dermatology, the correlation between intestinal microbial agents and cutaneous lesions is well established. Giardia duodenalis, an intestinal parasite, has been particularly widely studied. The aim of this work is to provide a review of studies demonstrating the involvement of G. duodenalis in various forms of dermatosis.Patients and methodsThe data were obtained by an English-language literature search of Medline, PubMed and Google Scholar for the period 1975–2015. Among the thirty case reports since 1976, we selected the twenty most objective and clinically relevant.Results and discussionThis review demonstrates that intestinal giardiasis may be an etiological factor, either alone or in combination with other agents, of various dermatoses through inflammatory and allergic mechanisms or intestinal hyperpermeability. The mucocutaneous lesions are varied: urticaria, angioedema, atopic dermatitis, erythema nodosum, Wells syndrome, among others. The role and origin of the infection are often unknown, and it is thus difficult to determine the interval between parasite infestation and the onset of skin lesions. Consequently, a fecal examination to identify G. duodenalis should be considered in chronic urticaria or angioedema, and where atopic dermatitis occurs in adulthood without any specific etiology. Therapeutic test should be done in every suspicion.
Self-testing for contact allergy to hair dyes – a 5-year follow-up multicentre study
Summary
Background
In 2011, a multicentre study was conducted in order to determine how hair dye manufacturers instructed consumers to perform a self-test prior to dyeing their hair, in order to identify individuals who are likely to react upon subsequent hair dyeing. A number of concerns were raised concerning the variability in instructions between products and producers, and the safety and validity of this tool.
Objectives
To perform a 5-year follow-up study in order to determine whether manufacturers still recommend a self-test, and if so, whether the procedures have been changed.
Methods
During March 2016, a total of 40 oxidative hair dye products from 21 different manufacturers were bought in retail stores in 8 European countries.
Results
The consumers were instructed to perform a self-test prior to hair dyeing for 39 of the products; however, the procedures varied greatly regarding the method of application, the amount of hair dye applied, the location and size of the application area, the number of applications, whether or not rinsing was performed after application, the reading times, and how a positive reaction was defined.
Conclusions
Self-testing is still recommended by almost all manufacturers of permanent hair dyes. There are major variations in the instructions, even in products from the same manufacturer. The previously raised concerns regarding safety and validity still remain.
Sleep quality and disturbances in patients with dystrophic epidermolysis bullosa

BackgroundSleep disturbances in patients with dystrophic epidermolysis bullosa (DEB) have never been investigated and still remain an open question. We sought to examine sleep quality and its relationship with pain and mood disorders in DEB patients.MethodsTwenty-eight DEB patients and 26 healthy individuals completed the following battery of scales: the Pittsburgh Sleep Quality Index (PSQI), the Epworth Sleepiness Scale (ESS), the Medical Outcomes Study (MOS) Sleep Scale, the Visual Analogue Scale (VAS), the Hamilton Rating Scale for Anxiety (HAM-A) and Depression (HAM-D). Linear correlations and hierarchical regression analyses were performed.ResultsPatients with DEB have significantly lower scores in daytime sleepiness (p = 0.003) and higher level of pain (p = 0.009) in comparison to controls. The analysis of all PSQI and MOS items failed to demonstrate any statistically significant differences between DEB patients and controls, except for a sub component of PSQI measuring sleep disturbances (p = 0.003). In the study group, a positive correlation was found between PSQI and depressive symptoms (r = 0.56; p = 0.002), anxiety symptoms (r = 0.46; p = 0.015), and pain (r = 0.44; p = 0.020). These results were consistent with the hierarchical regression analysis showing a significant contribution of depression (ΔR2 = 21.8; p = 0.008), anxiety (ΔR2 = 14.5; p = 0.027), and pain (ΔR2 = 13.5; p = 0.032) to poor quality of sleep.ConclusionsDEB patients showed a greater degree of sleep disturbances that can be influenced by depressive symptoms, anxiety symptoms, and pain compared with controls. Therefore, they should be screened for sleep issues that can be possibly impacted by pain and mood disorders.
Acné juvenil: intervención psicoeducativa en 334 adolescentes

IntroducciónEl acné juvenil es una condición muy prevalente en la población adolescente. A pesar de su alta prevalencia, es escaso el conocimiento que tienen los adolescentes de esta dermatosis. Entre las fuentes de información más utilizadas por los adolescentes destacan los amigos y los familiares. Supone una importante repercusión en la calidad de vida, con posibilidad de desarrollo de cuadros depresivos y aislamiento social.ObjetivosMejorar el conocimiento del acné de los alumnos preadolescentes y facilitarles herramientas psicológicas para una mejor relación mental con esta enfermedad.Material y métodosSe realizó un estudio experimental no controlado con adolescentes de 1.° de la ESO de 3 colegios de la Comunidad de Madrid. Se realizó un cuestionario preintervención recogiendo sus conocimientos y posicionamiento previo. Acudimos a los 3 colegios a explicarles una presentación con la fisiopatología, tratamientos y herramientas psicológicas.Posteriormente se les entregó un cuestionario postintervención pasadas 4 semanas.ResultadosN=334 estudiantes rellenaron el cuestionario preintervención y n=307 el postintervencional. Los cuestionarios no válidos no se incluyeron en el análisis estadístico (n=23 y n=16, respectivamente). La principal fuente de información son los familiares y amigos. El 50% señalan la falta de higiene como un factor clave en la aparición del acné, y un 18% considera que se contagia por contacto. Destaca la falta de conocimientos acerca de los tratamientos médicos disponibles.ConclusionesAunque se trata de una experiencia limitada, los resultados de nuestro estudio sugieren que las intervenciones psicoeducativas son una herramienta costo-eficiente, con muy buena aceptación, y útil para mejorar el posicionamiento frente a una enfermedad prevalente y con importante repercusión en la calidad de vida, como es el acné juvenil.IntroductionJuvenile acne is a very common medical condition among adolescents. Despite this high prevalence, adolescents have a remarkable lack of knowledge about this dermatosis, with friends and relatives being their most used information source. Acne has a significant impact on quality of life, and some patients may develop depressive symptoms and social isolation.ObjectiveTo improve the knowledge about acne of pre-adolescent schoolchildren, and to provide them with the psychological tools for a better approach to this condition.Materials and methodsA non-randomised experimental study was conducted on adolescents (12-14 years old) in 3 schools in the area of Madrid, Spain. A pre-interventional questionnaire was completed to determine their knowledge and attitude towards acne. Talks were given by two dermatologists in the 3 schools, and the topics covered were pathophysiology, treatments, and psychological tools. A post-interventional questionnaire was completed 4 weeks later so that the impact of the programme could be evaluated.ResultsA total of 334 students completed the pre-interventional questionnaire and 307 the post-interventional questionnaire. Questionnaires including nonsense answers were not considered for the statistical analysis (n=23 and n=16, respectively). Relatives and friends were found to be the main source of information about acne. Lack of personal hygiene was associated with acne by 50% of the participants, with 18% of the adolescents considering acne a contagious disease. It is remarkable the lack of knowledge about therapeutic options.ConclusionsAlthough this a limited experiment, the results of our study suggest that psycho-educational interventions are a well-accepted cost-effective tool that may change attitudes towards a very prevalent medical condition, and which can have a significant impact on quality of life.
